Participant Questionnaire

All who have experienced an out-of-body experience (OBE), regardless of whether or not they were close to death, are invited to be a part of this important study. To participate, please complete this questionnaire, providing as much detail as possible to each question. If you come upon a question about your experience in which you do not remember, please let us know this rather than guessing or assuming. Also, if you do not wish to answer a specific quesiton, you do not have to do so, but can just type in something indicating such to move to the next question. There are 25 sections with one or more questions, and the average time to complete this questionnaire is estimated to be a minimum of 30 to 45 minutes. Upon completing this questionnaire, you will have the opportunity to request that a free copy of the results of this phase of the study be made available for you to download (you will be notified after we are done with this phase of our study). If you have more than one experience you would like to submit, please do not answer the questions in a collective sense in which you mix all your experiences together. Please complete a new questionnaire for each additional experience. Thank you.

The only regisration requirement is your first name, email, and entering a password that you choose, and though the other personal information will be helpful if you choose to include it, it is not required to participate in this study. Any personal contact information you provide will never be used for advertising or marketing purposes. It also will never be given to others without your specific approval. YOU MUST BE 18 OR OVER TO PARTICIPATE IN THIS STUDY.
